Opening Plenary

Keynote Speaker: Wayne Centrone, Physician, Medical Director

Homelessness, Poverty and Health - The Web that has no Weaver
Dr. Centrone, a family practice physician from Portland, Oregon who has
dedicated his career to working with homeless and underserved
populations will offer a keynote address on the long- and short-term
consequences of poverty and homelessness on health.
Children and Homelessness
The presentation will provide information about the latest
research and best practices for preventing and ending homelessness
among families with children. It will describe opportunities and
recommendations for utilizing the Homelessness Prevention and Rapid
Re-Housing Program (HPRP).

SkanPoint
This workshop will demonstrate and discuss the SkanPoint feature
in ServicePoint. HALO received the first SkanPoint grant from the
Department of Commerce to implement the tool in their service, their
staff will discuss their reactions to the tool and explain their
findings.

SOAR
A panel of current Wisconsin SOAR (SSI/SSDI Outreach, Access, and
Recovery) providers will present their experiences with the SOAR
program.

Panel of Homelessness Veterans Services
A panel of veterans services experts will discuss the services
currently available for homeless veterans.

Supportive Housing 101
This session is designed to give participants a basic
understanding of the concept of permanent supportive housing. Topics
will include the definition, features, benefits, and examples of
supportive housing; how supportive housing fits into the housing
continuum; identifying target populations; range of housing options;
and available technical and funding resources.

HUD update by Sernorma Mitchell, Updates from the Wisconsin
Department of Commerce by Secretary Dick J. Leinenkugel and Homeless
Conference Awards Presentation
Homeless Youth: LGBT Youth Aged Out of Foster Care
This workshop will explore the impacts of homelessness for youth
of the lesbian, gay, bisexual, and transgender community.

HMIS Outcomes
Learn about ways to use Wisconsin ServicePoint to track outcomes
for your homeless programs.

Porchlight Products
This innovative program combines employment opportunities with
production of local sustainable ingredients into products for retail
sale.

Health and Homelessness
Dr. Cathering Ranheim will discuss barriers to good health in
medical underserved patients, including medical illiteracy, adequate
housing, lack of insurance, inadequate access to primary care,
transportation, psychiatric diagnoses and substance abuse issues.

Permanent Supportive Housing: A Funding How –To Guide
This workshop is targeted to project managers, staff, and board
members of organizations that are actively considering developing
supportive housing. The session will provide a discussion of key issues
in supportive housing development and finance, a checklist of tools and
tasks to be completed in the development process, and ample time for
questions and answers.

Housing First: A Rapid Re-Housing Model
This workshop will discuss how the program works here in Dane
County and the successes and challenges there have been along the way.

How to do a Homeless Client Count
Learn about practices and tools to make your Point-In-Time counts
more accurate. Basic methodologies for conducting sheltered and
unsheltered point-in-time counts will be presented. Through the case
study of Washington DC, the presentation will explore methods for
coordinating point-in-time counts across multiple jurisdictions, as
well as how to incorporate HMIS. Understand and implement the basic
mechanics of an accurate, compliant point-in-time count of sheltered
and unsheltered persons in Wisconsin.

Project Homeless Connect
Learn about Minnesota’s Project Homeless Connect and what
they have done to incorporate this form of outreach in their homeless
services programs. Milwaukee’s Continuum of Care is currently
incorporating this program into their ten year plan to end homelessness
and two of their members will also be on hand to discuss their work.

PATH
A panel of current Wisconsin PATH (Projects for Assistance in the
Transition from Homelessness) will discuss the unique qualities of each
of their programs.

Alfredson and Brad Munger
Funding Updates
This session will give a description of the changes to HUD's
McKinney-Vento homeless assistance programs known as the HEARTH Act as
well as other federal funding opportunities. An update of HPRP and
other Wisconsin programs will also be provided.

Closing Plenary - A Place to Call Home – The
Importance of Stability for Recovery
Speaker: Dr. David Mays, M.D., Ph.D.
Psychiatrist David Mays will present a summary of our best
understanding of how homelessness contributes to poor outcomes for
those with mental illnesses and how having a place to call home not
only improves the quality of life for those with psychiatric
disabilities, but also improves life in the community for everyone.
